The Parent/Guardian and Student sections of the Sports Physical Forms MUST be COMPLETED and SIGNED prior to your exam.
The Sports Physical Treatment Authorization form must also be completed and can be downloaded HERE!
The Sports Physicals will be offered at the Holzer Clinic in Jackson and is free of charge for students who will be in Grades 7 through 12 during the 2024-2025 School year.
