Immunizations
The Ohio Department of Health regulates the rules governing the immunization of children in Ohio.
Immunizations required by Section 3701.13 and 3313.671 of the Ohio Revised Code are ammended each school year to reflect any changes that have been implemented.
Requirements are evidence of:
*All Preschool students – Hepatitis B (3 in a series)
*All Preschool students – 1 MMR
*All Preschool students – 4 DTP/DTaP/DT/TD
*All Preschool students – 3 Polio
*All Preschool students - 1 Varicella (Chickenpox)
*All Kindergarten students - Hepatitis B (3 in a series)
*All Kindergarten students - 2 MMR
*All Kindergarten students – 5 DTP/DTaP/DT/Td if the fourth dose was administered prior to the fourth birthday
*All Kindergarten students – 4 Polio if the third dose was administered prior to the fourth birthday
*All Kindergarten students - Varicella (chickenpox) or written evidence from parent/guardian or physician that your child has had natural chicken pox
*All 7th grade students - Tdap Booster & One (1) dose of meningococcal vaccine must be administered prior to entry
*All 12th grade students - Two (2) doses of meningococcal vaccine must be administered prior to entry
*If the first dose of meningococcal vaccine was administered at 16th birthday or older, the second dose is not required.
Pupils who have not been immunized by “a method of immunization approved by the department of health pursuant to Section 3701.13 of the Revised Code,” are to be excluded from school attendance no later than fifteen days after admission.

Welcome to the 2023-2024 school year. We are the school nurses for the District. We can be reached at Central Elementary - 740-596-4386, South Elementary - 740-384-2731, West Elementary - 740-596-5236, the Middle School - 740-596-5243 and the High School - 740-596-5258. We can also be reached through the County Office at 740-596-5218 Ext. 236. In case of an emergency you may call 740-577-8081.
Your child's safety is very important to us. Policies and procedures are in place to protect all of our students and staff. Frequent handwashing and the use of hand sanitizer continues to be the most effect way to combat the spread of germs.
Immunizations must be up-to-date and records turned in the first day of school. Any student without updated immunizations will be sent home beginning the 16th day after school begins.
